PAYROLL: DUES CHECK OFF

As Discussed at the May 20th Membership Meeting:

Payroll companies have switched to paperless start-work in response to the COVID-19 Pandemic.

Local 764 is working to get Local 764 DUES CHECK OFF and Local 764 401(k) DEDUCTION FORMS included in these digital start-work packets.

Until this happens, you must turn in your deductions forms to your production payroll – either via email or in person and we strongly encourage you to send a COPY OF THIS FORM to the Local 764 office in care of David Erdei ([email protected]).  This simple act allows Local 764 to advocate for you with the payroll companies.

PDF versions – both fillable and non-fillable – are available on the website.  You may also request electronic/digital versions at anytime from the office.  If your payroll accountant would like a different format, please let us know!
